A carotid MRI is a noninvasive imaging test that uses magnetic resonance to visualize the carotid arteries in the neck. It shows artery wall structure, plaque, stenosis and blood flow without ionizing radiation. Often done with or without contrast, it helps assess stroke risk, guide treatment decisions, and monitor disease progression. It typically requires lying still inside an MRI scanner.

Common side effects: mild nausea, headache, dizziness, injection-site pain or bruising, warmth or metallic taste, and allergic reactions (rash, itching). Rare but serious: anaphylaxis and, in people with severe kidney disease, risk of nephrogenic systemic fibrosis. Small amounts of gadolinium may be retained in tissues; long-term significance is unclear. Tell providers about kidney problems, pregnancy, or prior contrast reactions.

Gadolinium-based contrast agents are most commonly used in MRI. Injected intravenously, they alter tissue magnetic properties (primarily shortening T1 relaxation) to enhance image contrast. Reactions are uncommon but allergic responses can occur; there is a small risk of nephrogenic systemic fibrosis in patients with severe kidney impairment, so kidney function is checked beforehand. Iron-based agents and non-contrast techniques are alternatives.

Warning signs of a blocked carotid artery include sudden weakness, numbness, or paralysis of the face, arm, or leg—often on one side; sudden confusion, slurred speech, or trouble understanding; sudden vision loss or transient monocular blindness; sudden dizziness, loss of balance, or trouble walking; and sudden severe headache with no known cause. These suggest stroke or TIA and need immediate medical attention.

Carotid scanning (Doppler ultrasound) detects narrowing or plaque in the carotid arteries to assess stroke risk, investigate symptoms such as transient ischemic attacks or unexplained neurological events, and guide treatment decisions (medical management, carotid endarterectomy or stenting). It's noninvasive, helps monitor plaque progression or post‑procedural results, and is used selectively in high‑risk or symptomatic patients to prevent ischemic stroke.

Carotid arteries are critically important because they supply oxygenated blood to the brain. Blockage from atherosclerosis or injury can cause transient ischemic attack, stroke, or death. Many problems are silent until symptoms appear. Early detection and treatment—lifestyle changes, blood‑thinning medication, or procedures like endarterectomy or stenting—significantly reduce risk.
