Contrast helps define lesion extent, activity, and blood–brain-barrier disruption for more accurate diagnosis and surgical planning.

Gadolinium-based contrast agents (IV gadolinium chelates, e.g., gadobutrol or gadoterate) are used for lumbar spine MRI when extra detail is needed — to highlight tumors, infection, inflammation, or nerve-root enhancement. They’re given intravenously; allergic reactions are uncommon. Contrast is avoided or used cautiously in severe renal impairment because of nephrogenic systemic fibrosis risk. However, conventional MRI shows anatomical and indirect signs, not nerve function or microscopic axonal damage. A full-spine MRI with contrast typically takes about 45–90 minutes overall. The actual imaging portion is usually 30–60 minutes, while added time for IV contrast injection, positioning, safety screening and post-scan monitoring expands the appointment. Exact duration varies by scanner type, the sequences required (e.g., cervical, thoracic, lumbar), and departmental workflow, so expect up to 90 minutes in some centers.
