MRI Gluteal Area

Yes—MRI is the preferred imaging test to visualize gluteal tendon and muscle tears. It shows partial- and full-thickness tendon tears, muscle injuries, retraction, atrophy, associated bursitis and fluid, and helps guide treatment. Ultrasound can detect some tears but is operator-dependent; MRI provides the most detailed assessment of extent and chronic changes.

The gluteal area is the buttock region at the back of the pelvis, spanning from the lower back/waistline down to the upper thigh. It covers the posterior hip and lateral pelvis, overlying the sacrum and greater trochanter. It contains the gluteal muscles—maximus, medius, and minimus—which support hip movement, posture, and weight-bearing during standing, walking, and climbing.

Trendelenburg test assesses gluteus medius/minimus dysfunction. With the patient standing, they lift one foot while the examiner observes the pelvis. A positive test shows pelvic drop on the unsupported side or a compensatory trunk lean toward the stance limb, indicating hip abductor weakness or gluteal tendinopathy. It helps localize lateral hip/gluteal pathology and guide further evaluation.

MRI cannot directly detect pain but is a highly sensitive test for many causes of hip pain. It shows bones, bone marrow, cartilage, labrum, tendons, ligaments, bursae, stress fractures, avascular necrosis, synovitis and tumors. MRI helps identify structural or inflammatory causes when X‑rays are normal. Findings must be interpreted with clinical history and exam, as imaging changes don’t always explain symptoms.
