In MRI, CVJ stands for craniovertebral junction. It denotes the region where the skull base (occiput) meets the upper cervical spine—mainly the atlas (C1) and axis (C2). MRI of the CVJ assesses congenital anomalies, trauma, inflammatory disease, tumors, and instability affecting the neural structures, ligaments, and nearby vascular relationships at this critical head–neck transition.

MRI provides superior soft‑tissue contrast and detail, better for tumors, demyelination, posterior fossa lesions, and subacute ischemia, and avoids ionizing radiation. Choice depends on clinical need: CT for immediate trauma/bleed evaluation; MRI for comprehensive, nonurgent brain assessment.

Craniovertebral junction (CVJ) surgery duration varies widely: straightforward posterior decompression often takes 2–4 hours; instrumented fusion or occipitocervical fixation typically requires 4–8 hours. Combined or anterior–posterior approaches and complex revisions can extend to 8–12 hours. Exact timing depends on surgical approach, deformity complexity, neuromonitoring, blood loss and patient health; anesthesia and positioning add extra perioperative time.

During a CVJ (craniovertebral junction) exam expect a focused history and physical: inspection and palpation of the neck, range-of-motion tests, neurological checks (strength, sensation, reflexes, gait, coordination), cranial nerve assessment, and provocative maneuvers. The clinician may order imaging (X‑ray, CT, MRI; sometimes flexion‑extension or vascular studies). The exam is noninvasive but may briefly reproduce symptoms.
