A brain MRI with contrast uses a gadolinium agent to highlight areas where the blood–brain barrier is disrupted. It makes tumors, metastases, infections, abscesses, active inflammatory or demyelinating lesions, and vascular abnormalities more conspicuous, helps distinguish scar from recurrent disease, and defines lesion size and enhancement patterns for diagnosis and treatment planning. It may miss microscopic changes.

Most imaging centers offer gadolinium-based contrast for brain MRI and use it when clinically indicated — for detecting tumors, infections, inflammation, or blood–brain barrier disruption. Contrast isn’t always necessary; your referring clinician will decide. Inform staff if you’re pregnant, breastfeeding, have kidney disease, or prior contrast reactions. Kidney function tests may be required beforehand and risks should be discussed with the radiology team.

MRI uses magnetic fields and is generally considered safe during pregnancy because it does not use ionizing radiation. Non-contrast MRI can be performed when clinically necessary at any stage, though many practitioners avoid routine scans in the first trimester unless essential. Gadolinium contrast is usually avoided unless the benefit outweighs potential risks. Discuss risks and timing with your clinician.

Gadolinium-based contrast for brain MRI is generally safe but carries small risks: allergic reactions, mild side effects (headache, nausea, injection-site pain), and rare nephrogenic systemic fibrosis in people with severe kidney impairment. A brain MRI can detect many neurological problems—tumors, strokes, bleeding, infections, demyelinating disease (e.g., multiple sclerosis), vascular abnormalities, and significant atrophy. Advanced MRI techniques (contrast, diffusion, angiography) improve detection. However, some conditions—early neurodegenerative changes, metabolic or functional disorders, and certain causes of headache—may appear normal on MRI. Normal imaging doesn’t rule out all neurological disease; further tests may be needed.
