Microfilaria Detection

Microfilaria detection is laboratory identification of the microscopic larval stage (microfilariae) of filarial parasites in blood, skin, or tissue samples to diagnose filarial infections. Methods include thick/thin blood smears, concentration techniques (e.g., Knott’s), Giemsa staining, antigen tests, and molecular assays like PCR. Timing of blood collection (night or day) depends on the species’ periodicity and improves detection.

A microfilaria test is a blood examination that detects microfilariae—the larval stage of filarial worms that cause lymphatic filariasis. It typically examines peripheral blood (often collected at night because of nocturnal periodicity) using thick/thin smears, concentration methods, or rapid antigen assays. Detection confirms active infection and helps guide antiparasitic treatment and public-health interventions.

Microfilaria testing by peripheral blood smear is timed to the parasite’s periodicity. For Wuchereria bancrofti, collect night blood—typically between 10:00 PM and 2:00 AM, with peak around midnight. Some filarial species or modern antigen/antibody assays do not require nocturnal sampling and can be performed anytime. Always follow the laboratory’s sample-collection instructions for optimal results.

The confirmatory test for filarial infection is demonstration of microfilariae in peripheral blood (night‑time thick and thin smears) or detection of circulating filarial antigen (CFA) by immunochromatographic tests or ELISA. PCR can confirm species and detect low-level infections. Choice depends on the suspected species and timing; CFA tests are commonly used for Wuchereria bancrofti.

Lymphatic filariasis (elephantiasis) is caused by microfilariae — the larval stage of filarial worms such as Wuchereria bancrofti, Brugia malayi and B. timori. Transmitted by mosquitoes, microfilariae migrate to and damage the lymphatic system, producing chronic lymphedema, recurrent infections and progressive swelling of limbs and genitals. Diagnosis uses blood tests; treatment includes antiparasitic drugs and supportive care.

The microfilaria (peripheral blood smear) test typically costs roughly ₹200–₹1,000 in India at private labs. In the UK it’s usually covered by the NHS; private testing can run about £30–£100. In the US, expect roughly $20–$150 depending on the lab, technique, and whether additional serology is included. Prices vary by location, lab, and public versus private testing.
