Malarial Antigen detection (Rapid)

The rapid diagnostic test (RDT) for malaria is a lateral‑flow immunoassay that detects Plasmodium antigens in a finger‑prick blood sample—commonly HRP2 (P. falciparum) and/or pLDH (all species). It gives results in about 15–20 minutes, is portable and simple to use. Limitations include variable sensitivity, false negatives from HRP2 gene deletions, and antigen persistence after treatment, so microscopy or repeat testing may be needed.

Rapid detection of malaria uses point-of-care rapid diagnostic tests (RDTs) that identify parasite antigens in a finger-prick blood sample, delivering results in 15–30 minutes without microscopy. RDTs are easy to use in clinics and remote settings, reliably detect Plasmodium falciparum and some other species, and guide prompt treatment; however, sensitivity varies and microscopy or PCR may be used to confirm or speciate infections.

To use a rapid malaria antigen test: check kit integrity and expiry, wash hands, perform a finger‑prick and collect the required blood volume, add blood to the sample well, then add the supplied buffer. Start the timer and read results at the manufacturer’s specified time (usually 15–20 minutes). A visible control line confirms validity; a test line indicates malaria antigen. If positive or invalid, repeat or seek confirmatory testing and medical care.

Malaria rapid diagnostic tests detect parasite antigens: histidine‑rich protein 2 (HRP2), mainly from Plasmodium falciparum; parasite lactate dehydrogenase (pLDH), which can be pan‑Plasmodium or species‑specific (Pf‑pLDH, Pv‑pLDH); and pan‑Plasmodium aldolase. Some tests combine HRP2 with pLDH or aldolase for broader detection and species differentiation; antigen persistence and HRP2 gene deletions can affect sensitivity.

Rapid antigen detection is necessary because it provides quick, low-cost identification of infectious individuals at point of care, enabling prompt isolation, treatment and contact tracing. Faster results relieve laboratory burden, expand testing access in community and low-resource settings, and help control outbreaks by identifying contagious cases during early symptomatic stages, guiding timely clinical and public health decisions.

To check a rapid antigen test: wash hands, confirm kit expiry, and follow the instructions. Collect the swab as directed, mix it in the extraction buffer, and add the required drops to the test well. Wait the specified time (usually 15–30 minutes). A visible control line confirms validity; a test line (even faint) plus control = positive, only control = negative. If no control line, invalid; if symptomatic and negative, repeat or get PCR.
