Magnetic Resonance Elastography (MRE) – Liver

MRI elastography of the liver is a noninvasive imaging test that measures liver stiffness to detect and stage fibrosis and cirrhosis. It combines low‑frequency mechanical vibrations with MRI to produce a quantitative “elastogram” map of tissue stiffness. MRE is accurate, reproducible, useful for monitoring disease or treatment response, and is safe with no ionizing radiation, working well in obese patients.

MR elastography costs vary by country and provider. In the US it typically adds roughly $300–$1,500 to imaging bills (total MRI+MRE about $500–$2,000); private UK clinics charge a fee while NHS patients receive it free; in India private prices often range around INR 3,000–12,000. Insurance frequently covers it when clinically indicated; exact fees depend on facility and region.

Liver MR elastography (MRE) is a highly accurate, noninvasive test for detecting liver fibrosis and cirrhosis, with sensitivity and specificity often above 80–90% for advanced fibrosis. It's more reliable than ultrasound elastography for staging significant disease but less sensitive for very early fibrosis. Results can be affected by inflammation, congestion, iron overload, and technical factors; still widely used as a biopsy-sparing tool.

A doctor orders a liver MRI to better detect and characterize focal lesions (benign versus malignant), stage liver cancers or metastases, evaluate fatty liver, cirrhosis or fibrosis, and assess vascular or biliary abnormalities (e.g., portal vein thrombosis, Budd–Chiari, bile duct obstruction). It’s used for pre‑surgical planning, monitoring treatment response, or when ultrasound/CT findings are inconclusive.

Liver elastography (e.g., transient elastography/FibroScan) primarily measures liver stiffness to assess fibrosis but can also detect and quantify fatty liver when combined with the Controlled Attenuation Parameter (CAP). It’s a noninvasive, widely used screening and monitoring tool, reasonably accurate for moderate-to-severe steatosis but less sensitive for mild fat; biopsy remains gold standard if diagnosis or staging is unclear.

Liver elastography is a non-invasive, generally painless scan. Most people feel only light pressure or brief discomfort as the transducer presses the skin and ribcage; the exam lasts minutes and requires no anaesthetic. Some with tender or enlarged livers may experience momentary pain, but serious complications are rare. Results assess liver stiffness without biopsy risks.
