Lipase

Elevated lipase usually signals pancreatic injury, most commonly acute or chronic pancreatitis. It often accompanies severe upper abdominal pain, nausea and vomiting, and may indicate complications like pancreatic necrosis, pseudocyst, infection or organ failure. Causes include gallstones, heavy alcohol use, certain medications, trauma or kidney dysfunction. High lipase warrants prompt medical evaluation, imaging and treatment of the underlying cause.

Lipase is a digestive enzyme—mainly produced by the pancreas and small intestine—that breaks dietary triglycerides into free fatty acids and glycerol for absorption and energy use. Pancreatic lipase is essential for fat digestion; other tissues produce lipases involved in fat metabolism. Blood lipase levels are measured clinically because elevated levels can indicate pancreatitis or other pancreatic disorders.

Normal adult serum lipase is typically about 0–160 units per liter (U/L), though many labs use ranges like 10–140 U/L; exact cutoffs vary by assay and laboratory. Use the reference range printed with your result. Markedly elevated lipase suggests pancreatitis or pancreatic injury and needs clinical correlation and further testing.

Elevated serum lipase most commonly indicates pancreatic inflammation or injury (pancreatitis). Pancreatic damage can impair insulin production, so acute pancreatitis may cause transient hyperglycemia and chronic or severe pancreatic disease can increase the risk of diabetes. High lipase alone does not diagnose diabetes; glucose testing (fasting glucose, HbA1c) and clinical evaluation are needed to assess diabetes risk.

Treatment for high lipase targets the underlying cause—most commonly acute pancreatitis. Initial care includes fasting, aggressive IV fluids, pain control, anti‑nausea meds and early enteral nutrition as tolerated. Specific therapy treats causes: ERCP or cholecystectomy for gallstones, lipid‑lowering for hypertriglyceridemia, stopping offending drugs. Severe cases require ICU support, monitoring, drainage or necrosectomy for infected necrosis.

Food itself rarely causes a true rise in serum lipase. Normal eating may raise pancreatic secretion but usually does not increase blood lipase significantly. Marked lipase elevations usually indicate pancreatic inflammation or injury (for example, pancreatitis from gallstones, alcohol, or very high triglycerides).
