Lead detected in urine indicates recent or ongoing lead exposure; it reflects what the body has recently absorbed and is excreting, not long-term bone stores. Elevated urinary lead prompts investigation to remove the exposure source, assessment for symptoms (neurological, gastrointestinal, hematologic, renal), and confirmatory testing with blood lead levels plus appropriate medical follow-up. Children and pregnant individuals are particularly vulnerable.

The urine lead test measures lead excreted in urine using techniques like atomic absorption spectroscopy or ICP‑MS. Collection can be a 24‑hour sample or a spot urine adjusted for creatinine; some tests use chelation to provoke excretion. Results are reported in µg/L or µg/g creatinine and help monitor chronic exposure or treatment response, though blood lead level testing is preferred for recent exposure.

Urine lead levels are usually very low and there’s no single universal “normal” cutoff because blood testing is preferred. In people without occupational exposure, urinary lead is generally <5–10 µg/L (or <30 µg/g creatinine). Higher values suggest recent exposure; occupational reference values are higher. Any detectable lead in children warrants medical evaluation and removal of exposure.

Lead is nephrotoxic: it accumulates in kidneys and damages proximal tubules, causing reduced glomerular filtration, proteinuria, glycosuria and aminoaciduria, and a Fanconi‑like syndrome in severe cases. Chronic exposure leads to interstitial nephritis, irreversible chronic kidney disease, reduced concentrating ability (polyuria/nocturia), and contributes to hypertension and progressive renal impairment. Acute high‑level exposure can cause acute tubular injury and hematuria.

Yes. Lead exposure can harm the kidneys: acute high levels and chronic low-level exposure cause tubular dysfunction, reduced glomerular filtration, proteinuria and chronic kidney disease. Children and adults with long-term exposure or high blood lead may develop hypertension and progressive renal impairment. Prevention involves eliminating exposure, monitoring blood lead and kidney function, and chelation therapy when medically indicated.

To remove lead, stop ongoing exposure and get blood lead testing. Significant lead levels may require medically supervised chelation therapy (for example succimer, EDTA, or dimercaprol). Supportive measures—hydration and nutrition with iron, calcium, vitamin C, and zinc—can reduce absorption and aid recovery.
