JC / BK Virus (JC / BK Polyoma virus) - Quantitative

The quantitative test for BK virus is a quantitative real-time PCR assay that measures BK viral DNA in blood (plasma/serum) or urine. Results are reported as viral load (copies/mL or IU/mL). Persistent plasma BK viral loads above approximately 10,000 copies/mL (≈4 log10) are associated with increased risk of BK virus nephropathy; serial monitoring guides transplant management.

BK virus can be very serious after a kidney transplant. In immunosuppressed recipients it may reactivate and cause BK virus–associated nephropathy, leading to progressive graft dysfunction and, if untreated, graft loss. It most often appears in the first year, is detected by PCR, and is managed primarily by reducing immunosuppression; early detection and monitoring are crucial to prevent irreversible damage.

Most polyomaviruses are common and usually harmless in healthy people, causing no symptoms. However, in immunocompromised patients they can be serious: JC virus can cause progressive multifocal leukoencephalopathy (PML), BK virus can damage transplanted kidneys, and Merkel cell polyomavirus is linked to a rare skin cancer. JC (John Cunningham) and BK viruses are human polyomaviruses that infect most people in childhood and establish lifelong latent infection in the urinary tract and kidneys. Reactivation in immunosuppressed patients causes serious disease: BK virus can cause nephropathy and hemorrhagic cystitis, while JC virus causes progressive multifocal leukoencephalopathy (PML). Diagnosis uses PCR; treatment centers on reducing immunosuppression and supportive care.

A positive BK virus test means BK viral DNA has been detected in urine or blood, indicating active infection or reactivation. It’s particularly significant in immunosuppressed people, especially kidney transplant recipients, because it can cause BK virus–associated nephropathy or hemorrhagic cystitis. Management includes close viral-load monitoring, reducing immunosuppression when safe, and specialist follow-up to protect kidney function.

HBV quantitative (HBV DNA) is reported in IU/mL. Undetectable is typically <20 IU/mL. Low viral load: 20–2,000 IU/mL; intermediate/active: 2,000–20,000 IU/mL; high: >20,000 IU/mL (often prompts treatment consideration if ALT is raised). In pregnancy, >200,000 IU/mL is a threshold for antiviral prophylaxis. Assays vary and should be interpreted by your clinician.
