Islet Cell Antibody

Islet cell antibodies include islet cell cytoplasmic antibodies (ICA), glutamic acid decarboxylase antibodies (GAD65), insulin autoantibodies (IAA), insulinoma‑associated‑2 antibodies (IA‑2), and zinc transporter 8 antibodies (ZnT8). These autoantibodies target pancreatic beta‑cell antigens, serve as markers of autoimmune destruction in type 1 diabetes, and can appear months to years before clinical onset, aiding diagnosis and risk assessment.

The four main autoantibodies associated with type 1 diabetes are: glutamic acid decarboxylase antibodies (GAD65), insulin autoantibodies (IAA), insulinoma‑associated antigen‑2 antibodies (IA‑2/ICA512), and zinc transporter 8 antibodies (ZnT8). These autoantibodies help diagnose and predict autoimmune destruction of pancreatic beta cells and may appear months to years before clinical onset.

Islet cell antibodies are also called islet cell cytoplasmic antibodies (ICA), anti‑islet antibodies, islet autoantibodies or pancreatic islet autoantibodies. Clinically the term often refers broadly to specific islet autoantibodies such as anti‑GAD (GAD65), insulin autoantibodies (IAA), IA‑2 antibodies (IA‑2A), and ZnT8 (zinc transporter 8) antibodies. These markers help diagnose and predict type 1 diabetes.

A positive IA-2 antibody test indicates an autoimmune response against pancreatic beta-cell protein IA‑2. It supports a diagnosis of type 1 diabetes or predicts higher risk of developing it, often correlating with faster progression to insulin deficiency. A positive result guides closer monitoring and earlier insulin-focused treatment planning; it does not indicate infection and isn’t typical of type 2 diabetes.

Islet autoantibodies indicate autoimmune attack on pancreatic beta cells. Presence of one or more (e.g., GAD65, IA‑2, ZnT8, insulin antibodies) supports type 1 diabetes or latent autoimmune diabetes in adults; multiple antibodies and higher titers mean greater risk of progression. A negative test doesn’t fully exclude autoimmune disease early on. Interpret results alongside symptoms, blood glucose and C‑peptide for diagnosis and prognosis.

IgG, IgM, IgA, and IgE. IgG is the most abundant blood antibody, provides long-term immunity, opsonizes pathogens, and crosses the placenta to protect the fetus. IgM is the primary, early responder forming pentamers to agglutinate microbes. IgA is secreted at mucosal surfaces and in breast milk to guard the gut and respiratory tract. IgE mediates allergies and parasite defense.
