IGG Type 4

IgG subclass 4 (IgG4) is one of four IgG antibody types and is normally the least abundant. It increases with prolonged or repeated antigen exposure and is relatively anti‑inflammatory because it poorly fixes complement and binds Fc receptors. Raised IgG4 can indicate chronic immune stimulation or the fibroinflammatory condition called IgG4‑related disease; mild isolated changes are often not clinically significant.

IgG4‑positive means there are elevated IgG4‑class antibodies in blood or increased IgG4‑producing plasma cells in tissue. This finding can suggest IgG4‑related disease, a fibroinflammatory disorder causing mass‑like lesions and organ dysfunction (for example, pancreas, salivary glands, bile ducts). IgG4 positivity alone isn’t diagnostic—clinical context, imaging, and tissue histology are needed to confirm diagnosis.

IgG4-related disease can be serious because it causes inflammation and progressive fibrosis in one or more organs, potentially leading to organ enlargement, dysfunction, or failure if untreated. It often responds to corticosteroids and other immunosuppressive treatments, but relapses and permanent scarring can occur. Early diagnosis and treatment reduce the risk of irreversible damage and long-term complications.

IgG4 antibodies aren’t simply good or bad — their effect depends on context. They act as anti‑inflammatory, “blocking” antibodies helpful in allergy and immunotherapy, reducing harmful immune reactions. However, high IgG4 levels can indicate IgG4‑related disease, a fibroinflammatory condition that damages organs. Clinical interpretation depends on symptoms, imaging and other tests, so elevated IgG4 needs medical evaluation.

IgG4 subclass autoimmune hepatitis is a variant of autoimmune liver inflammation marked by elevated serum IgG4 and liver infiltration by IgG4-positive plasma cells. It can overlap with IgG4‑related disease, presenting with fatigue, jaundice and raised liver enzymes and sometimes lacking typical autoantibodies. Diagnosis relies on liver biopsy and IgG4 testing; treatment is immunosuppression, usually corticosteroids, with generally good response.

IgG4-related disease is usually managed by a rheumatologist or clinical immunologist because it’s an immune-mediated, multisystem condition. Care is often multidisciplinary — gastroenterologists, pulmonologists, nephrologists, endocrinologists, otolaryngologists, and surgeons or radiologists may be involved depending on affected organs. Primary care doctors coordinate referrals and long-term follow-up alongside specialists for diagnosis, biopsy interpretation, and immunosuppressive treatment.
