HLA alleles are linked to many autoimmune and immune-mediated conditions: ankylosing spondylitis (HLA‑B27), celiac disease (HLA‑DQ2/DQ8), type 1 diabetes (HLA‑DR3/DR4–DQ8), rheumatoid arthritis (HLA‑DRB1 shared epitope), psoriasis (HLA‑Cw6), narcolepsy (HLA‑DQB1*06:02), and inflammatory bowel disease. Specific HLA types also predict drug hypersensitivity (e.g., abacavir, allopurinol) and affect transplant rejection and graft‑versus‑host risk.

Testing positive for HLA antibodies means your immune system has formed antibodies against human leukocyte antigens. It usually reflects prior sensitization (from pregnancy, transfusion or transplant) and can make organ transplantation harder by increasing rejection risk and limiting compatible donors. Further testing will identify specific or donor‑specific antibodies; management may include tailored donor selection, extra crossmatching, or desensitization.

HLA-A is a class I human leukocyte antigen on most nucleated cells that presents endogenous peptide fragments to CD8+ cytotoxic T cells, enabling recognition of infected or malignant cells. It’s used in tissue typing for organ and stem‑cell transplantation, influences susceptibility to certain autoimmune and drug hypersensitivity reactions, and plays a role in cancer immunity and responses to infections.

HLA testing identifies human leukocyte antigen variants to match organ, bone‑marrow and stem‑cell donors with recipients, guide tissue typing, and predict or help diagnose certain autoimmune conditions (for example HLA‑B27 in ankylosing spondylitis and HLA‑DQ2/DQ8 in celiac disease). It’s also used to identify genetic risk for immune‑mediated drug reactions (such as abacavir hypersensitivity) and in immunogenetic research.

HLA antibodies aren’t harmful by themselves in most healthy people, but they matter clinically. They indicate sensitization (from prior transplants, transfusions, or pregnancy) and can cause organ transplant rejection, reduce graft survival, and make finding compatible donors harder; they can also cause platelet transfusion failure. HLA-related diseases produce different symptoms depending on the specific disorder but commonly include chronic joint or lower-back pain and morning stiffness (as in spondyloarthropathies), eye inflammation (acute anterior uveitis), skin changes (psoriasis, rashes), mouth ulcers, chronic fatigue, gastrointestinal symptoms (abdominal pain, diarrhoea, weight loss in coeliac disease), and, sometimes, severe drug-reaction rashes and systemic symptoms.
