HRCT THORAX (Contrast)

HRCT chest is usually performed without intravenous contrast because it optimizes lung-parenchymal detail and assesses interstitial lung disease, fibrosis, bronchiectasis, and small airways. Contrast is generally unnecessary and can obscure fine lung markings; it may be used if vascular or mediastinal pathology is suspected or when combined with CT pulmonary angiography. Your clinician will decide based on the clinical question.

A CT thorax with contrast is a computed tomography scan of the chest performed after intravenous injection of an iodine‑based contrast dye. The contrast highlights blood vessels and improves detection of lung disease, tumors, infections, inflammation, and clots. The test involves lying on a table while images are taken and may require kidney‑function blood tests; rare allergic reactions to contrast can occur.

Costs vary by country and facility. In India an HRCT chest with contrast typically costs about ₹2,000–8,000. In the UK it’s free on the NHS but private scans run roughly £200–£700. In the US outpatient prices vary widely, roughly $300–$3,000 depending on hospital, contrast use, and whether insurance covers it. Final price depends on location, facility, and insurance.

HRCT thorax is generally safe but involves higher ionizing radiation than a chest X‑ray, slightly increasing lifetime cancer risk. If iodinated contrast is used, possible side effects include allergic reactions (mild itching to rare anaphylaxis) and contrast-related kidney injury in people with renal impairment. Other issues: discomfort from lying still, claustrophobia, bruising or pain at the injection site, and fetal risk in pregnancy.

Contrast is not always needed for a lung CT. Non-contrast low‑dose CT is used for lung cancer screening and most parenchymal disease. Contrast is added when assessing blood vessels (e.g., pulmonary embolism), staging tumors, or evaluating mediastinal/vascular anatomy. Kidney function and iodine allergy affect suitability. The choice depends on the clinical question—discuss the reasons, risks, and benefits with your clinician.

A normal HRCT thorax shows clear lungs with no focal opacities, consolidation, nodules, masses, or ground-glass changes. Interstitium, bronchi, vasculature and pleura appear normal without thickening, bronchiectasis, air‑trapping or lymphadenopathy. Cardiomediastinal contours are unremarkable and no acute disease is evident. Overall lung volumes and architecture fall within expected limits for the patient’s age and clinical context.
