HLA- T AND B CELL LYMPHOCYTE CROSS MATCH

The HLA crossmatch test checks whether a transplant recipient has antibodies that react to a donor’s HLA proteins. It identifies donor-specific anti-HLA antibodies using methods such as complement-dependent cytotoxicity, flow cytometry, or a virtual crossmatch based on antibody and HLA typing. A positive crossmatch signals high risk of hyperacute or acute rejection and guides donor selection and treatment.

Lymphocyte cross-matching is a pretransplant laboratory test that detects recipient antibodies against donor lymphocyte antigens (mainly HLA). It identifies preformed donor-specific antibodies that can cause hyperacute or acute rejection. Methods include complement-dependent cytotoxicity, flow cytometry, and virtual crossmatching. A positive crossmatch usually contraindicates transplantation or prompts desensitization to lower rejection risk.

The crossmatch test for kidney transplant checks whether the recipient has antibodies that react against the donor’s cells, mainly anti-HLA antibodies. Laboratory methods include complement-dependent cytotoxicity and more sensitive flow cytometry; a “virtual” crossmatch uses antibody and HLA typing data. A negative crossmatch indicates compatibility; a positive crossmatch predicts high risk of hyperacute rejection and usually rules out direct transplant or prompts desensitization.

The HLA test identifies human leukocyte antigen proteins and any anti‑HLA antibodies in donor and recipient before a kidney transplant. It assesses compatibility (HLA typing) and detects antibodies or a positive crossmatch that increase rejection risk. Results help choose the best donor, predict immunologic risk, and guide post‑transplant monitoring and immunosuppression to reduce graft rejection.

The most important single HLA to match is HLA‑DR (particularly DRB1), since mismatches here strongly increase rejection and affect graft survival. HLA‑A and HLA‑B are also important; for hematopoietic stem‑cell transplants, matching HLA‑A, ‑B, ‑C and HLA‑DRB1 (aiming for an 8/8 or 10/10 match including DQB1) is critical to lower graft‑vs‑host and rejection risks.

A crossmatch test confirms that a donor’s red blood cells are compatible with a recipient’s plasma by detecting antibodies that could cause a transfusion reaction. It verifies the absence of clinically significant recipient antibodies against donor red cells and helps ensure safe blood transfusion. Major and antiglobulin (indirect) crossmatches identify incompatible units and reduce the risk of hemolysis and reactions.
