HLA-DR Typing

HLA typing is a blood test that identifies a person’s human leukocyte antigen (HLA) proteins—genetic markers on white blood cells involved in immune recognition. It’s performed by analyzing DNA or blood cells to determine HLA variants. Results guide organ and bone marrow donor matching, assess risk for certain autoimmune conditions, and inform transplant compatibility and some medication decisions. Testing requires a simple blood draw.

HLA-DR is a human leukocyte antigen class II cell-surface protein on antigen-presenting cells (dendritic cells, macrophages, B cells). It presents peptide fragments to CD4+ T helper cells, initiating adaptive immune responses. HLA-DR typing helps with organ transplant matching and disease association studies; specific HLA-DR variants are linked to susceptibility to certain autoimmune conditions. Reduced or increased HLA-DR expression can indicate immune activation or suppression.

HLA-DR test identifies HLA‑DR, a major histocompatibility complex (MHC class II) molecule. It can mean HLA‑DR typing, which determines tissue‑type alleles for transplant compatibility and autoimmune risk, or an HLA‑DR expression assay (usually by flow cytometry) measuring HLA‑DR on monocytes/antigen‑presenting cells to assess immune function—used in sepsis monitoring, immunosuppression, and therapy guidance.

HLA typing associated with celiac disease detects HLA‑DQ2 and HLA‑DQ8. About 90–95% of patients carry HLA‑DQ2; most of the rest carry HLA‑DQ8. These alleles are necessary but not sufficient—many people with them never develop celiac disease. A negative HLA‑DQ2/DQ8 result makes celiac disease very unlikely and is often used when serology or biopsy are inconclusive.

HLA molecules are essential components of the immune system that help present antigens and distinguish self from foreign—generally beneficial. However, specific HLA alleles can increase risk of autoimmune disorders, certain drug hypersensitivities, and transplant rejection when donor and recipient HLA mismatch. So HLA is neither simply good nor bad; its effects depend on the particular alleles and clinical context.

HLA typing costs vary by country and test complexity. In the US, expect roughly $200–$1,200 for routine to high‑resolution testing; in the UK private fees are about £100–£400 (often NHS‑covered for clinical needs). In India prices range roughly ₹4,000–₹15,000. More comprehensive or NGS‑based typing costs more; insurance or transplant centers often cover clinical tests.
