HLA-B TYPING

Being HLA‑B positive means you carry one or more HLA‑B alleles, part of the class I human leukocyte antigen system on most cells. HLA‑B presents peptides to immune cells, influencing transplant compatibility, susceptibility to some autoimmune diseases (for example HLA‑B27 and ankylosing spondylitis) and the risk of certain drug hypersensitivities (such as HLA‑B*57:01 with abacavir).

HLA typing tests for specific variants of HLA (human leukocyte antigen) genes — proteins on cell surfaces that present antigens to the immune system. Results identify class I (HLA‑A, B, C) and class II (HLA‑DR, DQ, DP) alleles to match organ or stem‑cell donors, assess autoimmune disease associations, predict some drug hypersensitivities, and aid certain diagnostic, forensic, or paternity evaluations.

HLA-B alleles are linked to several diseases: HLA‑B27 strongly with ankylosing spondylitis, reactive arthritis, psoriatic and inflammatory bowel disease–associated arthritis; HLA‑B51 with Behçet’s disease; HLA‑B*57:01 predicts abacavir hypersensitivity; HLA‑B*58:01 predicts allopurinol‑induced severe cutaneous reactions (including SJS/TEN). HLA‑B8 is part of a haplotype associated with myasthenia gravis and certain endocrine autoimmune conditions.

There are thousands of HLA‑B types. The HLA‑B locus has over four thousand distinct genetic alleles documented worldwide, with hundreds of protein/serological variants. New alleles continue to be discovered as genetic typing expands, making HLA‑B one of the most polymorphic human genes; this diversity affects transplant matching, disease association studies, and immune responses.

Yes. HLA-B is a standard HLA class I gene at the MHC locus on chromosome 6, and virtually all people carry the HLA-B locus — usually two alleles (one from each parent). It is highly polymorphic (many different alleles), which affects immune responses and transplant compatibility. Rare null alleles may reduce or abolish protein expression, but the gene locus remains present.

Neither is universally worse; severity depends on disease extent, age at onset, and treatment. Rheumatoid arthritis mainly damages peripheral joints, causing erosion, deformity, and systemic complications (lungs, heart), potentially causing disability. Ankylosing spondylitis mainly affects the spine and sacroiliac joints, risking spinal fusion, reduced chest expansion, and uveitis. Early diagnosis and modern therapies greatly improve outcomes for both.
