HLA‑DR4 positive means you carry the HLA‑DR4 human leukocyte antigen variant. HLA molecules present antigens to immune cells; DR4 is associated with higher risk for certain autoimmune diseases (for example rheumatoid arthritis and some forms of type 1 diabetes) but does not guarantee disease. It is inherited, detected by blood or tissue typing, and may inform risk assessment, prognosis, or treatment alongside clinical context.

HLA typing costs vary widely by country, lab and test resolution. Low-resolution/single-locus tests typically run about $100–$500; high-resolution/full-panel molecular typing often costs $500–$2,500 or more. HLA-A and HLA-B are human class I major histocompatibility complex (MHC) genes encoding cell-surface proteins that present peptide fragments to CD8+ T cells. HLA‑DR3 is linked to several autoimmune diseases, predisposing individuals to organ-specific and systemic autoimmunity. Common associations include type 1 diabetes, systemic lupus erythematosus, Graves' and Hashimoto's thyroid disease, myasthenia gravis, Sjögren's syndrome, celiac disease (often via the DR3‑DQ2 haplotype), autoimmune hepatitis, and autoimmune (primary) adrenal insufficiency (Addison disease). Strength of association varies by allele subtype and population.

Stage 4 rheumatoid arthritis is end-stage disease with extensive, irreversible joint destruction and deformities (ankylosis), severe pain, and profound loss of function and mobility. It commonly leads to marked disability, muscle wasting, and increased systemic complications. Treatment focuses on pain control, preventing complications, rehabilitation, assistive devices, and sometimes joint replacement or fusion; disease-modifying therapy aims to control residual inflammation.
