Hepatitis Core Antibody Total (HBcAb- Total)

Hepatitis B core antibody (anti‑HBc total) detects total antibodies (IgM and IgG) to the virus’s core antigen. A positive result indicates past or current infection — IgM suggests recent/acute infection, IgG indicates prior or chronic infection. It does not indicate immunity (that’s anti‑HBs). Positive anti‑HBc usually prompts further testing (HBsAg, anti‑HBs, HBV DNA) to clarify infection stage.

Hepatitis B core antibody total detected means you have antibodies to the virus’s core protein, indicating a current or past infection. The test detects both IgM (recent/acute infection) and IgG (past or chronic infection). It is not produced by vaccination. Additional tests — HBsAg, anti‑HBs and HBV DNA — are needed to determine infectivity, stage and immunity.

A normal HBcAb (hepatitis B core antibody) result is negative/non‑reactive, indicating no evidence of current or past hepatitis B infection. Many labs report HBcAb qualitatively; quantitative values and cutoffs vary by assay. IgM HBcAb suggests recent infection, while IgG HBcAb indicates past or resolved infection. Consult your lab report or clinician for specific interpretation.

If you’re anti‑HBc (HBcAb) positive, get tested for HBsAg and anti‑HBs first. If anti‑HBs is positive you’re immune and don’t need vaccination. If HBsAg is positive you have active infection and don’t get the vaccine—follow clinical care. If anti‑HBs negative and HBsAg negative, check HBV DNA; if DNA is negative, a vaccine series is recommended and response should be checked.

Hepatitis B "core positive" means a blood test detects antibodies to the hepatitis B core antigen (anti‑HBc). It indicates past or current infection; IgM anti‑HBc suggests recent/acute infection, while total anti‑HBc can persist for life after recovery. An isolated core‑positive result may reflect a resolved infection, the window period, or occult infection — further tests (HBsAg, anti‑HBs, HBV DNA) guide diagnosis and management.

Anti-HBc itself is not treated. Management depends on infection status: if resolved (anti-HBc positive, HBsAg negative), no antivirals—monitor and check anti-HBs; if acute, care is usually supportive, with antivirals in severe cases. If chronic HBV (HBsAg positive), long-term antivals such as tenofovir or entecavir are used. Immunosuppressed patients may need antiviral prophylaxis and HBV DNA/LFT monitoring.
