Hepatitis C Virus - Qualitative

A qualitative HCV test detects hepatitis C virus RNA in blood using nucleic acid amplification (PCR) to indicate whether active infection is is present. It reports “detectable” or “undetectable” and is used to confirm infection after a positive antibody screen or to document viral clearance. It does not quantify viral load; a separate quantitative PCR measures amount of virus.

The normal result for an HCV quantitative (HCV RNA) test is undetectable — i.e., below the assay’s limit of detection, commonly <15 IU/mL. Any detectable HCV RNA indicates current active infection; viral loads in untreated infection can range from hundreds to hundreds of millions of IU/mL. Different labs use different lower limits, so review your lab’s reference range.

Hepatitis C infection can be serious: while acute infection may be mild or asymptomatic, most untreated cases become chronic and can cause progressive liver inflammation, cirrhosis, liver failure, and hepatocellular carcinoma. Early diagnosis and modern direct-acting antiviral treatment can cure over 95% of cases and greatly reduce complications. Testing, avoiding blood exposure, and prompt treatment are important.

HCV qualitative PCR "not detected" means the test did not find hepatitis C viral RNA in the blood sample. This indicates no active HCV replication at or above the assay’s detection limit — consistent with cured or spontaneously cleared infection. It can also reflect testing during the early window period or very low-level viremia, so repeat testing is advised after recent exposure or during treatment monitoring.

If you test positive for HCV antibodies, you’ll need a confirmatory HCV RNA test to see if infection is active. If RNA-positive, clinicians assess liver damage (blood tests, imaging/fibrosis scoring) and usually offer highly effective direct-acting antiviral treatment that can cure >90–95%. Untreated chronic HCV risks cirrhosis and liver cancer. You’ll also get advice on avoiding alcohol, vaccinations (A/B), and partner testing.

HCV IgG qualitative is a blood test that detects immunoglobulin G antibodies to hepatitis C virus, indicating prior exposure. A positive result shows past or possible current infection but cannot distinguish active from cleared infection; follow-up HCV RNA testing is required to confirm active viraemia. A negative result may occur during the early window period before antibodies develop.
