Hemoglobin Electrophoresis

Hemoglobin electrophoresis identifies and measures different hemoglobin types (HbA, HbA2, HbF and abnormal variants) to diagnose and distinguish hemoglobin disorders. It detects conditions such as sickle cell disease and trait, beta‑thalassemia and many variant hemoglobins (e.g., HbC, HbD, HbE). It’s used for unexplained anemia, abnormal newborn screening, or when there’s a family history of hemoglobinopathy.

Normal adult hemoglobin electrophoresis typically shows HbA about 95–98.5%, HbA2 around 2–3.5%, and HbF under 1–2% (often <1%). Newborns normally have predominantly HbF. Significant deviations—such as low HbA with raised HbA2 or HbF, or presence of abnormal hemoglobin bands—suggest hemoglobinopathies and should prompt further clinical evaluation and correlation with symptoms and family history.

Hemoglobin electrophoresis separates hemoglobin types by mobility. Normal adults: mainly HbA, HbA2 <3.5%, HbF <2%. Beta‑thalassemia trait shows raised HbA2 (>3.5%) and sometimes increased HbF. Sickle trait: both HbA and HbS; sickle cell disease: predominantly HbS with little/no HbA and usually raised HbF. HbC/E produce distinct variant peaks. Correlate with CBC, clinical features, and genetic testing; recent transfusion can obscure results.

Yes. Hemoglobin (Hb) electrophoresis is a primary test for diagnosing thalassemia and other hemoglobinopathies. It identifies abnormal hemoglobin fractions; elevated HbA2 and HbF suggest beta‑thalassemia trait or intermedia, while alpha‑thalassemia carriers may appear normal. Results are interpreted with CBC indices and iron studies; DNA testing or prenatal molecular assays may be needed for definitive diagnosis.
