Fasting urine sugar should normally be absent—routine dipstick tests are negative for glucose. Any detectable glucose (trace or positive) is abnormal and suggests glycosuria, often reflecting blood glucose above the renal threshold (≈180 mg/dL) or renal causes. If urine glucose is present, further evaluation (fasting blood glucose, HbA1c, kidney assessment) is recommended.

During pregnancy, screening typically occurs at 24–28 weeks (earlier if high-risk). First is a 50 g glucose challenge: nonfasting drink, blood draw after 1 hour. If abnormal, a diagnostic oral glucose tolerance test (fasting; 75 g or 100 g) measures blood at fasting and at set intervals (1 and 2–3 hours). Follow fasting/medication instructions and expect monitoring if elevated.

Finding glucose in urine during pregnancy is relatively common because pregnancy raises blood glucose and lowers the kidneys’ threshold for spilling sugar. Mild, transient glucosuria isn’t always pathological, but persistent or large amounts warrant evaluation for gestational diabetes with formal blood glucose testing. Your clinician will assess symptoms, repeat tests, and may arrange an oral glucose tolerance test if needed.

Glycosuria—glucose in the urine—occurs when blood glucose exceeds renal reabsorption capacity or when kidney tubules fail to reabsorb glucose. In children it most often signals diabetes mellitus (especially type 1). It can also result from renal glycosuria or Fanconi syndrome (proximal tubular defects), transient hyperglycemia from illness or high carbohydrate intake, certain medications (e.g., SGLT2 inhibitors), or kidney disease.

Any detectable glucose in urine is abnormal and may suggest diabetes if persistent. Glycosuria usually occurs when blood glucose exceeds the renal threshold (~180 mg/dL or ~10 mmol/L). Urine dipstick grades roughly: trace (~50 mg/dL), 1+ (~100), 2+ (~250), 3+ (~500), 4+ (~1000 mg/dL). Diagnosis requires blood tests (fasting glucose, OGTT, or HbA1c).

To confirm sugar (glucose) in urine, provide a clean midstream urine sample for a reagent-strip (dipstick) test or laboratory urinalysis. If positive, repeat testing and follow up with blood glucose measurements (fasting or random) and HbA1c to confirm systemic hyperglycemia. Discuss findings with a clinician — dipsticks can be misleading and diagnosis requires blood testing and medical assessment.
