Fungal Stain

A fungal stain is a laboratory technique that uses special dyes or reagents on clinical specimens (skin, nails, hair, sputum or tissue) to reveal fungal elements—hyphae, yeast cells, spores—under the microscope. Common methods include KOH preparation, Gram, PAS, GMS and calcofluor white. These tests help detect, identify and guide treatment of fungal infections. Results are often rapid and support clinical diagnosis.

Gomori methenamine silver (GMS) is widely considered the most sensitive and reliable histochemical stain for detecting fungal elements in tissue. Periodic acid–Schiff (PAS) is also excellent for highlighting fungal cell walls. For rapid clinical detection, KOH preparation with calcofluor white fluorescence provides quick, sensitive visualization. Choice depends on specimen type and diagnostic needs.

Several stains detect fungi. In histology, Periodic acid–Schiff (PAS) highlights fungal cell walls magenta and Grocott’s methenamine silver (GMS) stains fungi black. For clinical specimens, potassium hydroxide (KOH) clears debris for direct microscopy; calcofluor white binds chitin and fluoresces; lactophenol cotton blue is used for culture mounts. India ink is used as a negative stain for Cryptococcus neoformans.

Several stains are used to visualize fungi: lactophenol cotton blue for culture mounts; calcofluor white with KOH for fluorescent detection of fungal cell‑wall chitin; periodic acid–Schiff (PAS) and Gomori methenamine silver (GMS) for histopathology; India ink as a negative stain for Cryptococcus capsules; Gram and methylene blue may also aid identification.

The principle of fungal staining is selective binding of dyes to fungal cell wall components (chitin, glucans and other polysaccharides) and internal structures to create contrast with the background. Specific stains—e.g., calcofluor white, lactophenol cotton blue, PAS and GMS—attach to these components or fluoresce, while fixation preserves morphology, enabling visualization of hyphae, spores and diagnostic features under the microscope.

First isolate the item and brush off dry spores outdoors. For washable fabrics, launder in hot water with detergent and oxygen bleach or add white vinegar; sun‑dry. For non‑washables, spot‑treat with hydrogen peroxide or diluted vinegar, let sit, blot and air. For hard surfaces, scrub with a diluted bleach solution (1 cup bleach per gallon water) or white vinegar, rinse and dry. Wear gloves, a mask and ventilate.
