Fluid for malignant cell identification

The "fluid for malignant cells" test examines body fluids (pleural, peritoneal, pericardial, or cerebrospinal) for cancerous cells. A sample is collected by aspiration and sent for cytology, often with cell‑block preparation and immunostaining. It helps diagnose, stage, or monitor malignancy. A positive result indicates malignant cells in the fluid; a negative result does not completely exclude cancer, so further testing may be needed.

Malignant cells are identified by abnormal features: pleomorphism (variation in cell and nuclear size/shape), high nucleus-to-cytoplasm ratio, hyperchromatic irregular nuclei, prominent nucleoli, frequent and atypical mitoses, tumor necrosis, and rapid proliferation, loss of normal differentiation (anaplasia), and evidence of invasion or metastasis. Diagnosis relies on biopsy histopathology, supported by immunohistochemistry, molecular testing, and imaging.

Malignant fluid is fluid that accumulates in body cavities (such as the chest or abdomen) and contains cancer cells. It develops when tumors invade serous linings or obstruct lymphatic drainage, causing breathlessness, cough, abdominal swelling, or pain. Diagnosis relies on imaging and cytological analysis of the fluid. Management aims at symptom relief—drainage, catheters, pleurodesis—and treating the underlying cancer to reduce recurrence.

Collect fluid aseptically into a sterile, leak‑proof container, label with patient details and clinical history, and submit promptly. Provide as much volume as feasible (eg, 20–50 mL for pleural/ascitic fluids; 50–100 mL urine). Do not add fixative unless the lab requests it. Refrigerate at 4°C if transport will be delayed, except cerebrospinal fluid, which should be sent immediately at room temperature.

Cytology fluid is a special preservative/transport medium used to collect and stabilize cells from body fluids or swabs for microscopic examination. It fixes cellular morphology, prevents decay and contamination, and may contain alcohol-based fixatives or proprietary buffers (e.g., for Pap tests and liquid-based cytology). Samples like pleural, peritoneal, urine, CSF, or cervical smears are placed in cytology fluid for laboratory processing and evaluation.

The definitive test to confirm malignancy is a tissue biopsy examined by a pathologist (histopathology). Biopsy types include core-needle, excisional, incisional and fine-needle aspiration (which can be less conclusive). Histology, often with immunohistochemistry and molecular tests, identifies cancer type and grade. Imaging and blood tests help guide diagnosis and staging but cannot alone confirm malignancy.
