Estrogen receptor (ER) and progesterone receptor (PR) are cellular proteins that bind estrogen and progesterone, acting as hormone-activated transcription factors. Their presence—commonly measured in breast tissue—indicates whether growth is hormone-driven and helps guide treatment. ER/PR-positive cancers often respond to hormonal therapies that block hormone action or lower hormone production, improving prognosis and informing treatment choices.

ER/PR-positive breast cancer is generally a favorable finding because tumor cells have estrogen and/or progesterone receptors and are more likely to respond to hormone therapies (e.g., tamoxifen, aromatase inhibitors). This often means more treatment options and a better prognosis than receptor-negative cancers. However, ER/PR-positive tumors still require appropriate treatment and can recur, so individualized management is essential.

ER/PR (estrogen- and progesterone-receptor) immunohistochemistry costs vary by country and provider. The PR marker is the progesterone receptor, a protein measured in breast cancer cells by immunohistochemistry. PR positivity (commonly defined as ≥1% of tumor cell nuclei staining) indicates a hormone-receptor–positive tumor and predicts likely benefit from endocrine therapies (e.g., tamoxifen, aromatase inhibitors). PR status, reported as percent staining or Allred score, also provides prognostic information when interpreted with estrogen receptor (ER) status.

About 70–80% of breast cancers are hormone receptor–positive, meaning they express estrogen receptors and/or progesterone receptors. These tumors often respond to endocrine therapies such as tamoxifen or aromatase inhibitors. Testing for ER and PR status is standard at diagnosis because it guides prognosis and treatment decisions. The proportion can vary by age, ethnicity and tumor subtype.

ER/PR‑positive, HER2‑negative breast cancer generally has a favorable prognosis. When detected early, it is often curable with surgery, radiation and endocrine (hormone) therapy, sometimes plus chemotherapy.
