Estimated Average Glucose (eAG)

Normal eAG is about 68–114 mg/dL (3.8–6.3 mmol/L), corresponding to an HbA1c of roughly 4.0–5.6%. Values above about 117 mg/dL (A1c 5.7%) indicate prediabetes; higher values indicate diabetes. eAG converts HbA1c into an average glucose level to compare with home readings; individualized targets may differ for people with diabetes.

A healthy average blood glucose is about 70–99 mg/dL (3.9–5.5 mmol/L) fasting and under 140 mg/dL (7.8 mmol/L) two hours after eating. For people with diabetes, common targets are 80–130 mg/dL before meals and under 180 mg/dL after meals; an average corresponding to HbA1c <7% is roughly 154 mg/dL, individualized by age and health.

If expressed in mmol/L, an eAG of 6.5 means your estimated average blood glucose is about 117 mg/dL. This corresponds to an HbA1c of roughly 5.7%, which sits at the threshold between normal and prediabetes. Estimated average glucose (eAG) translates HbA1c into an approximate average blood glucose and is useful for patient understanding and trends. However, it’s an estimate and can be inaccurate when HbA1c is affected by conditions like anemia, hemoglobin variants, pregnancy, kidney disease, or altered red‑cell turnover. Individual glucose variability and measurement methods (SMBG/CGM) can differ from eAG, so use it alongside glucose readings and clinical judgment.

If eAG is high it indicates your average blood glucose has been above target, increasing risk of diabetes complications (nerve, kidney, eye, heart). Act by confirming with an HbA1c test and more frequent glucose monitoring, and consult your healthcare team. They may recommend medication adjustments, carbohydrate management, weight loss, increased physical activity, and diabetes education to lower glucose and prevent complications.

For people without diabetes at any age, a normal HbA1c is under 5.7% (prediabetes 5.7–6.4%; diabetes ≥6.5%). In people with diabetes, targets are individualized: children/adolescents often aim ~7.0–7.5%; healthy adults generally <7.0%; older adults (≥65) may have relaxed goals, commonly 7.5–8.5% or higher if frail or with multiple comorbidities.
