Positive desmoglein‑3 means your blood contains autoantibodies against the desmoglein‑3 protein, commonly indicating pemphigus vulgaris—especially mucosal-predominant disease. A positive result supports the diagnosis and often correlates with disease activity and severity. It’s not definitive alone: clinical exam, biopsy, and specialist assessment are required. ELISA testing is commonly used and serial titers can help monitor treatment response.

Desmoglein 3 is a cadherin-family desmosomal adhesion protein expressed chiefly in stratified squamous epithelia and mucous membranes. It mediates calcium-dependent intercellular adhesion between keratinocytes, maintaining tissue integrity and mechanical strength. Dsg3 also contributes to intracellular signaling that regulates cytoskeletal organization. Autoantibodies against Dsg3 disrupt adhesion, causing acantholysis and blistering disorders such as pemphigus vulgaris.

Desmoglein 1 (Dsg1) is concentrated in desmosomes of the superficial epidermis of the skin and supports outer-layer keratinocyte adhesion. Desmoglein 3 (Dsg3) predominates in desmosomes of deeper epidermal layers and is richly expressed in mucosal epithelia (oral cavity, conjunctiva, esophagus). Both are desmosomal cadherins in epithelial tissues that mediate strong cell–cell adhesion.

Desmogleins are calcium‑dependent cadherin proteins that form desmosomal cell–cell junctions in epithelial and cardiac tissues. They mediate strong mechanical adhesion, maintain tissue integrity and barrier function, and participate in intracellular signaling for tissue organization. Different isoforms (e.g., DSG1 and DSG3) have layer-specific expression in skin and mucosa. Autoantibodies targeting desmogleins cause blistering diseases such as pemphigus.
