Dengue IgM

IgM positive in dengue indicates a recent or acute dengue infection. Dengue-specific IgM antibodies typically appear about 3–5 days after symptom onset, peak within two weeks, and can persist for weeks to months. A positive IgM suggests current or recent infection but may cross-react with other flaviviruses; confirm diagnosis with clinical assessment and additional tests (NS1 antigen, PCR, or IgG serology).

A positive IgM test means your immune system has recently encountered a germ and produced early-stage antibodies, suggesting a current or recent infection. IgM usually appears soon after exposure, before IgG. However, false positives, cross-reactions, or prolonged IgM can occur, so results should be interpreted with symptoms and clinical findings and may require confirmatory testing (repeat antibody or molecular tests).

IgM and IgG are different antibody classes. IgM is the first responder in acute infection, usually a pentamer that efficiently agglutinates pathogens and activates complement; its presence often indicates recent infection and it is relatively short‑lived. IgG is smaller, more abundant, appears later, provides long‑term immunity and memory, crosses the placenta, and specializes in neutralization and opsonization.

A negative dengue IgM means either no recent dengue infection or that testing was done too early for antibodies to appear. In early illness, NS1 antigen or PCR can detect virus; repeat IgM after 5–7 days if suspicion remains. False negatives may occur with low test sensitivity or immunosuppression. Clinical assessment and repeat or alternative testing guide diagnosis and management.

Dengue IgM antibodies typically become detectable about 3–5 days after symptom onset, rise to peak levels around two weeks, and remain detectable for roughly 2–3 months. In many patients IgM declines thereafter, though low-level positivity can persist for up to about six months in some cases. Timing varies with individual immune response and test sensitivity.

Dengue IgM indicates recent infection. Recovery focuses on rest, plenty of fluids (oral rehydration), paracetamol for fever and pain, and avoiding NSAIDs or aspirin because of bleeding risk. Get regular blood counts to monitor platelets and hematocrit and seek urgent care for severe signs (sustained vomiting, severe abdominal pain, bleeding, drowsiness, breathlessness, cold clammy skin). Prevent mosquito bites and avoid strenuous activity.
