Cytomegalovirus IgM Antibody

A CMV IgM antibody is an immune protein indicating recent or active infection with cytomegalovirus. IgM typically appears early after exposure and suggests a recent or acute infection, though it can sometimes persist or give false positives. Because timing matters—especially in pregnancy—positive IgM is usually followed by IgG testing, IgG avidity, or PCR to confirm infection timing and guide management.

If CMV tests positive, interpretation depends on the test: IgG alone usually means past infection and immunity; IgM or a detectable viral PCR suggests recent or active infection. Active CMV can cause serious disease in newborns (risk of congenital infection) and in immunocompromised people. Further testing (PCR, IgG avidity, fetal ultrasound in pregnancy) and specialist follow-up are recommended; antivirals may be used in severe cases.

IgM positive means your immune system has recently encountered an infection or antigen and is mounting an early antibody response. IgM appears soon after exposure, often indicating a current or recent infection, but can sometimes give false positives or reflect recent vaccination. Additional tests (IgG, repeat serology, or molecular testing) and clinical correlation are usually needed to determine timing and whether infection is active.

CMV IgM typically appears within 1–2 weeks after infection and is commonly detectable for about 2–3 months. In many people it wanes by 3–6 months, though low-level IgM can persist (or recur) for up to 6–12 months in some cases. Because IgM can persist or reappear with reactivation, IgG serology and avidity testing help distinguish recent from past infection.

CMV (cytomegalovirus) is usually mild or asymptomatic in healthy children and adults, often causing flu-like signs. However, it can be serious for unborn babies (congenital CMV may cause hearing loss and developmental disabilities) and for immunocompromised people (transplant recipients, advanced HIV), where it can cause organ-threatening disease. Primary infection during pregnancy and reactivation pose greatest risks.

Cytomegalovirus spreads through direct contact with infected body fluids—saliva, urine, blood, semen, vaginal secretions, and breast milk. It can pass from a pregnant person to the fetus, via sexual contact, blood transfusion, or organ transplantation, and through close contact in childcare settings. Many infections cause no symptoms, but the virus can reactivate or cause serious illness in newborns and immunocompromised people.
