Cytomegalovirus DNA PCR - Qualitative

CMV qualitative PCR is a molecular test that detects cytomegalovirus DNA in clinical samples (blood, plasma, urine, saliva, or amniotic fluid). It gives a positive/negative result to diagnose CMV infection—useful in pregnancy, neonates, transplant recipients, and immunocompromised patients—but does not quantify viral load. Quantitative PCR is used to monitor severity and treatment response.

A positive CMV PCR indicates cytomegalovirus DNA is present in the tested sample, showing active viral replication. It can reflect primary infection, reactivation, or congenital infection depending on context. Quantitative PCR (viral load) helps gauge severity and guides treatment, especially in immunocompromised patients or pregnant women. In healthy individuals it may be asymptomatic; interpret with clinical findings and other tests.

A CMV DNA quantitative PCR test measures the amount of cytomegalovirus genetic material in blood or other body fluids. It detects active infection, assesses viral load, and monitors response to antiviral therapy—especially in immunocompromised patients, transplant recipients, or during pregnancy. Results are reported as copies/mL or IU/mL and help guide treatment decisions and prognosis by tracking changes in viral levels over time.

A positive CMV DNA test (PCR) means cytomegalovirus genetic material was detected, indicating active infection or viral reactivation with ongoing replication. Viral load quantification helps assess severity and need for antiviral therapy, especially in pregnancy, newborns, or immunocompromised patients. In healthy adults it can be asymptomatic. Clinical assessment, repeat testing, and specialist guidance determine treatment and monitoring.

If cytomegalovirus (CMV) test is positive, it can indicate past exposure (CMV IgG) or recent/active infection (CMV IgM or rising IgG). Most healthy people have mild or no symptoms. Active CMV in pregnancy risks congenital infection; further tests and repeated serology or PCR are needed. When CMV PCR is positive, management depends on symptoms and immune status. Asymptomatic immunocompetent patients usually need no treatment—monitor. Symptomatic or immunocompromised patients require antivirals: oral valganciclovir for mild–moderate disease or IV ganciclovir for severe/organ‑invasive disease. Reduce immunosuppression if possible, adjust doses for renal function, monitor viral load and blood counts, and consider foscarnet/cidofovir for resistance.
