CT Scan Whole Abdomen Contrast With Urography

A CT scan of the whole abdomen with contrast is an X‑ray–based imaging test where an injected contrast dye improves visualization of abdominal organs, blood vessels and tissues. It produces detailed cross‑sectional images to evaluate the liver, kidneys, pancreas, spleen, bowel and vessels for tumors, infection, inflammation, stones, bleeding or vascular issues. Preparation may include fasting and checking kidney function before dye use.

Common effects after a CT urogram with iodinated contrast include brief warmth or flushing, a metallic taste, mild nausea, headache, dizziness, and injection-site bruising or pain. Allergic reactions (hives, itching) occur occasionally; severe anaphylaxis is rare. Contrast can impair kidney function in susceptible patients (contrast-induced nephropathy). Rare complications include extravasation at the IV site and delayed skin rashes.

Usually you should avoid solid food for 4–6 hours before a CT urogram; clear fluids (water, black tea/coffee without milk) are often allowed until about 1–2 hours beforehand. You may be asked to drink oral contrast at specific times. Tell the imaging team about diabetes, kidney problems, and all medications (especially metformin). Always follow the specific instructions given by the center.

CT urography itself is not usually painful. Lying on the table is generally comfortable, but IV insertion and contrast injection can cause brief stinging, a warm flushing sensation, metallic taste, or nausea. Some people feel anxious or claustrophobic. Serious allergic reactions are rare. Any sharp pain likely stems from the underlying condition, so tell staff about allergies, kidney issues, or pregnancy.

CT contrast risks include allergic reactions (mild hives to rare anaphylaxis), kidney injury—especially in people with preexisting kidney disease or diabetes—local injection problems (pain, swelling, extravasation), and potential thyroid dysfunction from iodinated contrast. There’s also additional radiation exposure. Rare delayed reactions and special precautions apply in pregnancy. Discuss prior allergies, kidney disease, and pregnancy with your clinician.

An abdominal CT can detect cancers of the liver (primary and metastases), pancreas, kidneys (renal cell carcinoma), adrenal glands, stomach and bowel/colon, ovaries and uterus, and less commonly the spleen. It also identifies retroperitoneal and mesenteric lymph node metastases, peritoneal carcinomatosis and abdominal soft-tissue or retroperitoneal tumors, and helps stage and monitor known abdominal malignancies.
