CT Scan Spine Lumbar Contrast

A CT scan of the lumbar spine with contrast is a specialized X‑ray exam that creates detailed cross‑sectional images of the lower back after intravenous iodinated contrast is given. The contrast highlights blood vessels and abnormal tissue, improving detection of tumors, infection, inflammation, vascular lesions, and post‑operative changes, and helps distinguish scar from recurrent disc disease. Risks include allergic reaction and kidney effects; kidney function is checked first.

Contrast is usually not required for routine CT of the spine—noncontrast CT is preferred for trauma, fracture, and bony assessment. IV contrast is added when there is concern for tumor, infection/abscess, or to evaluate enhancing soft-tissue or vascular lesions; CT myelography (intrathecal contrast) is an alternative when MRI is contraindicated. MRI remains superior for neural and soft-tissue evaluation.

Before a lumbar spine CT: bring ID and any prior imaging, wear loose clothing and remove jewelry/metal. You may change into a gown. Tell staff if pregnant, diabetic, allergic to contrast, or have kidney problems. If contrast is used you may need to fast and have blood tests. Continue most medications unless told otherwise. You’ll lie still during the quick scan.

A non-contrast lumbar CT scan usually involves a short imaging time—typically 5–15 minutes of actual scanning. Including arrival, positioning on the table and brief instructions, the whole appointment commonly lasts about 20–30 minutes (sometimes up to 45 minutes if the department is busy). Images are available immediately but the radiologist’s formal report may take several hours to a few days.

Wear loose, comfortable clothing without metal (no zippers, snaps, underwire bras or metal buttons). You may be asked to change into a hospital gown. Remove jewelry, piercings, watches, belts and any removable metallic objects; leave contact lenses out if advised. Bring easy-to-remove shoes. Tell staff about implants, pacemakers or pregnancy before the scan.

A CT scan reliably shows bony spinal injuries — fractures, alignment problems, bone spurs and spinal canal narrowing — and is commonly used in acute trauma. It’s less sensitive for spinal cord, discs, ligaments and nerve-root injuries; MRI is preferred for soft-tissue, disc herniation or cord damage. CT myelography can help show nerve compression when MRI isn’t possible.
