COVID Antibody Count

There’s no universally accepted “good” COVID antibody level. Tests and assays vary, and protection depends on variant and time since vaccination or infection. Higher antibody titers generally mean greater protection; some studies cite about 264 BAU/mL (binding antibody units/mL) as correlating with roughly 80% protection against symptomatic disease, but thresholds aren’t definitive. High COVID antibody levels usually mean recent exposure or a strong immune response — most commonly recent vaccination/booster or recent infection. Hybrid immunity (infection plus vaccination) and more severe illness can produce higher titres. Different lab assays measure different antibodies and use varying scales, so results aren’t directly comparable. Discuss your results with a clinician, since antibody levels don’t perfectly predict protection and timing matters.

Normal ranges vary by test and lab. Most antibody assays provide a manufacturer-defined cutoff: results below the assay cut-off are reported as negative (commonly an index <0.8–1.0), borderline/equivocal around the cut-off, and positive above it. Quantitative tests report units (AU/mL or BAU/mL); higher values indicate a stronger response, but no single universal “protective” threshold exists.

COVID antibodies: IgM typically appears within 1–2 weeks after infection and declines over weeks to months. IgG emerges around 2–3 weeks, peaks in 3–7 weeks, and can remain detectable for at least 6–12 months, though levels often wane over time. Antibody persistence varies by severity, age, and vaccination; cellular immunity (memory B/T cells) can last longer than circulating antibodies.

Antibody tests report whether specific antibodies (usually IgM and/or IgG) are detected and sometimes their level. A positive IgM suggests recent exposure; IgG suggests past infection or vaccination. Negative results may mean no prior exposure, testing too early, or a weak response. Quantitative values must be interpreted against lab cutoffs; presence of antibodies doesn't guarantee immunity. Having COVID antibodies generally means past infection or vaccination and usually provides some protection, especially against severe illness. However, antibody levels and their duration vary, and protection isn’t complete—reinfection and variant escape can occur. Antibody presence doesn’t replace vaccination or boosters, particularly for older or immunocompromised people.
