What is a COT Test?
COT measures cotinine, a stable breakdown product of nicotine, in body fluids. Cotinine levels show recent use of tobacco, e-cigarettes, or nicotine replacement. It is useful because cotinine stays detectable longer than nicotine itself. Doctors use it to confirm smoking status, check for secondhand smoke exposure, and monitor quit-smoking programs. It helps assess risk tied to smoking, such as heart and lung disease, and to guide counselling and treatment. The test is also used in pregnancy care, workplace testing, and pre-surgical checks. Results must be interpreted with a history of nicotine product use, timing of exposure, and possible use of patches, gum, or vaping.

Why Take a COT Test?
COT is commonly included in smoking exposure or cessation panels and prenatal screening. Doctors may order it when a patient reports symptoms related to smoking, when verifying self-reported tobacco use, or to monitor progress in quitting programs. It helps diagnose recent tobacco or nicotine exposure and assess secondhand smoke risk. Abnormal results come from active smoking, vaping, nicotine replacement products, or heavy passive exposure. Family history of smoking-related disease may prompt testing to better manage risk.
