Cortisol - Spot Urine

Urinary cortisol measures how much cortisol the body excretes over 24 hours. Elevated levels suggest excess cortisol production (such as Cushing’s syndrome, severe stress, or certain medications), while low levels may point to adrenal insufficiency. Results can be affected by illness, pregnancy, or drugs. Abnormal findings usually lead to repeat testing and further evaluation to identify the cause and guide treatment.

Cortisol and its metabolites are eliminated in urine; measuring 24‑hour urinary free cortisol (UFC) is a standard way to evaluate cortisol production and help diagnose conditions like Cushing’s syndrome. Single spot urine tests can detect cortisol or cortisone but are less reliable. Results may be influenced by kidney function, certain medications, illness and stress.

Cortisol, the stress hormone, can contribute to breakouts. Elevated cortisol increases oil production, inflammation, and may disrupt the skin barrier and hormone balance, making acne more likely and slower to heal. Chronic stress often triggers flares and can worsen existing acne; addressing stress, sleep, skincare, and medical treatment can help reduce cortisol-related breakouts.

Urine cortisol rises with increased cortisol production or intake. Common causes include physiological stress (acute illness, surgery, intense exercise), Cushing’s syndrome from pituitary or adrenal tumors or ectopic ACTH production, and use of systemic glucocorticoid medications. Pregnancy, obesity, depression, heavy alcohol use, and some drugs that affect steroid metabolism can also elevate urine cortisol levels.

High cortisol can cause persistent fatigue, insomnia, mood changes (anxiety, irritability, depression), and trouble concentrating. Physical signs include abdominal weight gain, rounded “moon” face, fat at the upper back, thin skin, easy bruising, purple stretch marks, muscle weakness, and acne. It may also raise blood pressure and blood sugar, causing increased thirst and urination.

Urine cortisol is usually measured using a 24‑hour urine free cortisol test. Collect all urine for a full 24 hours: discard the first morning void, then save every sample including the first void the next morning. Keep the container refrigerated or on ice during collection and return it promptly to the lab. Your clinician may request repeat collections or other tests if needed.
