It’s most useful during the acute phase (first 3–7 days) when viral levels are high, providing early and specific confirmation of infection. Samples are usually serum; a negative result later may occur as antibodies develop, so serology may be needed.

Chikungunya is confirmed by laboratory testing: RT‑PCR (viral RNA) during the acute phase—typically within the first 5–7 days of symptoms—provides direct confirmation. After about day 5, chikungunya‑specific IgM antibodies on serology indicate recent infection. Plaque reduction neutralization test (PRNT) or virus isolation can serve as definitive confirmatory tests when cross‑reactivity with other arboviruses is a concern.

PCR detects chikungunya viral RNA and is most sensitive during the acute phase (about the first week of symptoms), confirming active infection. IgM serology detects the host antibody response that usually appears around 4–7 days after onset and can persist for weeks to months, indicating recent infection. PCR is best early; IgM helps later but may show cross-reactivity or timing-related limitations.

If chikungunya is positive, rest, hydrate and use paracetamol for fever and pain; avoid NSAIDs until dengue is excluded. Seek medical care for high fever, severe joint swelling, bleeding, or neurological symptoms. Protect against mosquito bites to prevent spread. Most recover in weeks, but persistent joint pain may need follow-up, physiotherapy or specialist care; pregnant women should consult their clinician.
