Chikungunya - IgM

Chikungunya IgM is the early immune antibody produced in response to chikungunya virus. Detecting IgM in blood indicates a recent or current infection, typically appearing about 3–7 days after symptom onset, peaking around 2–3 weeks, and declining over weeks to months. IgM testing helps diagnose recent infection and is often used alongside PCR or IgG tests to confirm timing.

IgM positive means your immune system has recently encountered an infection or antigen and is producing early-phase antibodies, suggesting current or recent infection. IgM typically appears days–weeks after exposure and falls as IgG rises. Results require clinical context because cross-reactions and false positives can occur; follow-up testing (repeat serology for IgG seroconversion or molecular tests) and clinical correlation determine significance.

IgM positivity indicates recent or current dengue infection; IgG positivity shows past exposure or a later-stage immune response. Both positive most often means a recent secondary dengue infection (or a later phase of a primary infection), which can raise the risk of severe disease due to antibody-dependent enhancement. Clinical assessment, confirmatory tests (NS1/PCR), and close medical monitoring are recommended.

Chikungunya IgM antibodies usually appear about 4–7 days after symptom onset, peak within the first few weeks, and generally remain detectable for about 2–3 months. In many patients IgM can persist longer, sometimes up to 4–6 months, although detection beyond three months is less common. Persistent IgM does not always indicate active infection.

If chikungunya is positive, expect supportive care: rest, plenty of fluids and acetaminophen for fever and pain. Avoid NSAIDs until dengue is excluded. Seek medical review if pregnant, a newborn, elderly, immunocompromised, or if you have severe symptoms (high fever, severe joint pain, bleeding, breathlessness, confusion). Protect against mosquito bites to prevent spread. Persistent joint pain may need specialist follow‑up.

There is no "IgM virus." IgM is a class of antibody produced early in an immune response. Finding pathogen-specific IgM in blood usually indicates a recent or acute infection, appearing days to weeks after exposure and waning over weeks to months. IgM tests can give false positives or negatives, so diagnosis is often confirmed with PCR, repeat testing, or IgG serology.
