Ceruloplasmin

High ceruloplasmin usually reflects an acute‑phase response or increased estrogen effect rather than primary copper disease. It can rise with inflammation, infection, pregnancy, oral contraceptives or hormone therapy, and some malignancies. Elevated levels are nonspecific; clinical assessment and additional tests (serum copper, inflammatory markers, and liver function) are needed to determine the underlying cause.

Low ceruloplasmin is treated by diagnosing and addressing the cause: test for Wilson’s disease and liver disorders, evaluate nutrition and copper status. If deficiency is confirmed, correct with dietary changes and supervised oral copper supplements; stop excess zinc or other interfering agents. Manage underlying liver or genetic conditions with specialist-directed therapy. Follow-up blood tests and hepatology/hematology consultation are essential.

Ceruloplasmin is a liver‑made copper‑carrying ferroxidase that transports most circulating copper and aids iron oxidation. In Wilson disease ATP7B mutations prevent proper copper incorporation into ceruloplasmin and reduce biliary copper excretion, producing low serum ceruloplasmin and release of toxic free copper that accumulates in liver, brain and cornea. Low ceruloplasmin is a common diagnostic clue but may be normal in some cases.

Ceruloplasmin abnormalities occur in several conditions. Low levels are seen in Wilson disease, aceruloplasminemia, Menkes disease, nephrotic syndrome and severe malnutrition. Elevated ceruloplasmin is an acute‑phase reactant in infections and inflammatory disorders (eg, rheumatoid arthritis), during pregnancy or with estrogen/oral‑contraceptive use, and in some malignancies. Measurement aids diagnosis and monitoring of these disorders.

Vitamin C does not increase ceruloplasmin. Typical dietary vitamin C has no meaningful effect on ceruloplasmin levels; some studies show high‑dose ascorbic acid can modestly lower serum copper and ceruloplasmin or reduce its oxidase activity. If you’re monitoring copper or ceruloplasmin for a health condition, avoid large vitamin C megadoses and discuss supplementation and testing with your clinician.

Wilson disease is an autosomal recessive disorder of copper metabolism (ATP7B mutation) causing copper accumulation in liver, brain and eyes, leading to hepatic failure, neurological signs, psychiatric symptoms and Kayser‑Fleischer rings. Lab findings include low ceruloplasmin, elevated urinary copper and increased hepatic copper. Prompt diagnosis and chelation (penicillamine, trientine) or zinc therapy prevents progression.
