CD5 (T Cell / B Cell Subset)

CD5 is primarily a T‑cell surface glycoprotein expressed on most peripheral T lymphocytes, but it is also present on a subset of B cells (CD5+ B‑1a cells). It modulates antigen‑receptor signaling and is used diagnostically to identify lymphoid populations and certain disorders, such as CD5‑positive B cells in chronic lymphocytic leukemia.

CD5-positive B cells are B lymphocytes that express the CD5 protein on their surface. Normally they form a small subset (B‑1a) involved in innate-like antibody production and immune regulation. Expanded populations occur in conditions such as chronic lymphocytic leukemia and some lymphomas. CD5 expression is detected by flow cytometry and helps classify B‑cell types for diagnosis and prognosis.

CD20 is a surface protein expressed on B cells (from late pre-B through mature B cells) and is not found on T cells or on antibody-secreting plasma cells. It helps regulate B-cell activation and is a common therapeutic target. Clinically, anti-CD20 drugs deplete B cells to treat certain lymphomas, leukemias, and autoimmune disorders.

CD5 can be a useful prognostic marker in specific lymphoid cancers but is context-dependent. In chronic lymphocytic leukemia CD5 positivity aids diagnosis but doesn’t alone predict outcome; in diffuse large B‑cell lymphoma CD5 expression often signals poorer prognosis. Overall, CD5 is best used alongside clinical, genetic, and other immunophenotypic data rather than as a sole prognostic determinant.

CD8+ T cells recognize peptide antigens presented on MHC class I molecules and mediate killing of virus-infected or malignant cells via cytotoxic mechanisms (perforin and granzymes). They function in cellular immunity rather than antibody production.
