CD38 (Plasma Cell / Prognostic CLL Marker)

CD38 is a surface glycoprotein measured on chronic lymphocytic leukemia (CLL) cells by flow cytometry. Its expression level serves as a prognostic marker: higher CD38 expression is associated with more aggressive disease, faster progression, and shorter time to treatment. CD38 status, alongside factors like IGHV mutation and cytogenetics, helps clinicians estimate prognosis and guide monitoring and therapy choices.

The most important prognostic factor in chronic lymphocytic leukemia (CLL) is TP53 disruption — del(17p) or TP53 mutation. These changes predict more aggressive disease, poor response to standard chemoimmunotherapy and shorter survival, often prompting targeted therapies. Other useful prognostic markers include IGHV mutation status, cytogenetics, lymphocyte-doubling time and clinical stage, but TP53 disruption has the greatest impact.

CD38 is a surface glycoprotein and ectoenzyme expressed on plasma cells, activated T and B cells, NK cells and others. It regulates NAD+ metabolism and calcium signaling, influencing cell activation, adhesion and differentiation. Clinically it is used as a diagnostic/prognostic marker (e.g., multiple myeloma, chronic lymphocytic leukemia) and as a therapeutic target for monoclonal antibodies in plasma-cell malignancies.

CD38-positive plasma cells are mature antibody-secreting B cells that express the surface glycoprotein CD38. They live in bone marrow and tissues, produce immunoglobulins, and support humoral immunity. High CD38 expression helps identify plasma cells in diagnostics and is seen in conditions like multiple myeloma; CD38 also has enzymatic and signaling roles and serves as a therapeutic target.

CLL prognosis is determined by clinical staging (Rai or Binet), symptoms and performance status, blood counts and lymphocyte doubling time, laboratory markers such as beta‑2 microglobulin, and detailed genetic/cytogenetic testing (FISH and mutation analysis including del(17p), TP53, del(11q) and IGHV mutation status). Age, comorbidities and treatment response also strongly influence outcomes and management.

CD38 and CD138 are surface markers on plasma cells with different roles. CD38 is a multifunctional ectoenzyme broadly expressed on activated lymphocytes and highly on plasma cells; it’s a therapeutic target in myeloma. CD138 (syndecan‑1) is a heparan sulfate proteoglycan more specific to mature, bone‑marrow plasma cells, mediating adhesion and survival and commonly used to identify terminally differentiated plasma cells in diagnostics.
