CD19 (Pan B Cell Marker)

CD19 is a transmembrane protein and a pan–B-cell marker expressed throughout B-cell development from pro-B cells to mature B cells, but typically lost when B cells differentiate into plasma cells. It is widely used to identify and classify normal and malignant B-cell populations in blood, bone marrow and lymphoid tissues, and serves as a target for B-cell–directed therapies.

CD19-positive means cells express the CD19 protein on their surface, identifying them as B lymphocytes. In clinical testing (flow cytometry or immunohistochemistry) CD19 positivity indicates B‑cell lineage, helping diagnose and classify B‑cell leukemias and lymphomas, monitor disease burden or minimal residual disease, and select or evaluate therapies that target CD19 (for example, monoclonal antibodies or CAR‑T cells).

A pan B‑cell marker is a surface or lineage protein expressed broadly across B lymphocyte stages, used to identify B cells in diagnostics and guide therapies. Common pan‑B markers include CD19 and CD20 (widely used for flow cytometry and targeted treatments); CD79a and PAX5 are additional lineage markers useful in pathology, with varying expression on plasma cells.

CD19 is a transmembrane co-receptor expressed through B‑cell development that amplifies B‑cell receptor (BCR) signaling by recruiting kinases and lowering the activation threshold. It forms a complex with CD21 and CD81, supporting B‑cell activation, proliferation, differentiation, antibody responses, and survival. CD19 is also used as a lineage marker and a therapeutic target in B‑cell malignancies and immunotherapies.

CD19 is expressed on most B‑cell lineage malignancies: B‑cell acute lymphoblastic leukemia (B‑ALL), chronic lymphocytic leukemia (CLL), and many non‑Hodgkin lymphomas (diffuse large B‑cell, follicular, mantle‑cell, Burkitt, marginal‑zone). It’s also found in hairy cell leukemia and precursor B‑cell lymphoblastic lymphoma. CD19 is generally absent or low on mature plasma‑cell myeloma and is a common immunotherapy target.
