Cardiac MRI evaluates heart structure, function and tissue characteristics. It diagnoses cardiomyopathies (dilated, hypertrophic, restrictive, ARVC), myocarditis and pericarditis, ischemic heart disease by detecting infarction, scar and viability, congenital heart defects, valvular abnormalities, cardiac tumors or thrombi, and aortic disease. It also assesses ventricular volumes, ejection fraction, myocardial perfusion and fibrosis to guide treatment and prognosis.

Cardiac MRI does not directly visualize most coronary artery blockages. It assesses heart structure, function, scar, and blood flow; stress perfusion MRI can detect ischemia or infarction that suggests significant coronary disease. Coronary MR angiography exists but is less commonly used and less accurate for detecting luminal stenosis than coronary CT angiography or invasive coronary angiography, which remain gold standards.

Complex studies (stress tests or additional sequences) can extend scan time. Your imaging center will give exact timing based on the protocol and whether contrast or stress imaging is required.

Cardiac MRI isn’t categorically better than echocardiography—each suits different needs. Echo is first-line: inexpensive, portable, and excellent for valve assessment and bedside hemodynamics. Cardiac MRI gives superior accuracy for chamber volumes, ejection fraction, tissue characterization (scar, inflammation), viability, and complex congenital or cardiomyopathy evaluation. Use MRI when echo is inconclusive or detailed quantification/tissue characterization is required; MRI is costlier and less available.

Yes—most people are awake during a cardiac MRI. It's a noninvasive scan where you lie still inside the scanner and follow breathing instructions; you’ll hear loud noises and can communicate with the technologist. An IV contrast may be given and heart/oxygen monitoring is used. Sedation or general anesthesia is reserved for claustrophobia, severe anxiety, young children, or inability to lie still.
