Cadmium - URINE

Cadmium in urine results from exposure to cadmium — chiefly cigarette smoke and ingestion of contaminated food or water (rice, leafy vegetables, shellfish), and occupational contact (battery manufacturing, smelting, welding, mining, metalworking). Environmental pollution, contaminated soil or fertilizers, and certain consumer products can contribute. Urinary cadmium reflects cumulative body burden and kidney damage, so elevated levels often indicate chronic exposure or renal injury.

The urine cadmium test measures cadmium concentration in a spot or 24‑hour urine sample (often reported as µg/g creatinine). Laboratories use sensitive methods (ICP‑MS or atomic absorption) to detect exposure and body burden; 24‑hour collections are more accurate. Results above reference ranges suggest significant exposure or kidney accumulation and warrant workplace/environmental assessment and medical follow‑up.

Chronic cadmium exposure causes Itai‑itai disease, a severe toxic condition marked by intense bone pain, osteomalacia, fractures, and progressive renal tubular damage that leads to proteinuria and kidney dysfunction. Inhaled cadmium also raises lung‑cancer risk, and long‑term exposure contributes to osteoporosis, hypertension, and increased cardiovascular disease risk. Primary sources include contaminated food, water, and industrial emissions.

Cadmium is a toxic heavy metal that accumulates in kidneys and liver. Chronic exposure damages kidney tubules causing proteinuria and impaired function, weakens bones (osteoporosis, fractures), and can cause lung damage and increased lung cancer risk when inhaled. It’s a human carcinogen and may harm the cardiovascular and reproductive systems and developing fetus. Symptoms often appear after long-term exposure.

Cadmium cannot be rapidly removed once absorbed. First step is stopping exposure (quit smoking, avoid contaminated food or workplace sources). Treatment is mainly supportive—hospital care for acute poisoning, monitoring kidney and bone health—and chelation has limited benefit and is reserved for select severe cases; dialysis is usually ineffective. Adequate iron, calcium and zinc intake can reduce intestinal cadmium absorption and aid long-term management.

Foods that can be high in cadmium include shellfish (oysters, mussels), organ meats (kidneys, liver), certain fish, cereal grains and rice (especially brown rice or rice from contaminated areas), leafy vegetables (spinach), root vegetables, and seeds and nuts (sunflower seeds, cocoa, Brazil and hazelnuts). Cadmium levels vary by soil, water and processing; chronic exposure mainly comes from diet and smoking.
