A normal total bile acid level is generally considered less than 10 µmol/L. In pregnancy, levels of ≥10 µmol/L suggest intrahepatic cholestasis; 10–39 µmol/L is mild-to-moderate, while ≥40 µmol/L denotes more severe disease and higher fetal risk. Total bile acids (TBA) measure the combined concentration of bile acids circulating in blood—primary and secondary bile acids made by the liver and modified by gut bacteria. Bile acids help digest and absorb fats, regulate cholesterol and metabolic signaling, and undergo enterohepatic circulation. Elevated TBA suggest impaired bile flow or liver dysfunction (eg, cholestasis, hepatitis) and are used diagnostically and in pregnancy monitoring.

If bile acids are high it indicates cholestasis or impaired bile flow from liver disease, bile-duct obstruction, drugs, or pregnancy-related intrahepatic cholestasis. Symptoms include itching, jaundice, pale stools, and fat malabsorption. High levels need prompt medical evaluation with liver tests and imaging, and treatment of the cause (medications like ursodeoxycholic acid, procedures, or early delivery in pregnancy) to reduce complications.

A bile acid test measures bile acid levels in blood to assess liver and bile duct function and detect impaired bile flow (cholestasis). It helps diagnose and monitor liver diseases, bile duct obstruction, and bile acid malabsorption, and is used in pregnancy to evaluate intrahepatic cholestasis. Results guide further testing and treatment decisions related to jaundice, pruritus, fat malabsorption, and liver injury.

Bile acids are neither simply good nor bad: they’re essential for fat digestion, absorption of fat‑soluble vitamins, and metabolic signaling, but excess or altered bile acids can harm the gut and liver. High or misplaced bile acids cause diarrhea, contribute to gallstones and bile duct injury, and may raise colon cancer risk when repeatedly altered. Balance and proper liver/bile flow determine benefit versus harm.

Severe bile acid levels are generally defined as total serum bile acids ≥100 µmol/L. Levels above 10 µmol/L are abnormal; 40–99 µmol/L are usually classed as moderate. In pregnancy, levels ≥100 µmol/L carry a substantially higher risk of fetal complications and prompt more urgent monitoring and management. Clinicians may recommend earlier delivery or treatment when severe levels are detected.
