Beta-2- Glycoprotein -1 IgM

A positive beta‑2 glycoprotein I (β2‑GPI) IgM indicates antiphospholipid antibodies that can be associated with antiphospholipid syndrome (APS). Isolated or low‑titre IgM may be transient after infection; persistent moderate–high titres on repeat testing (≥12 weeks) plus clinical events (thrombosis, recurrent pregnancy loss) support APS. It raises clotting and pregnancy‑risk and needs repeat testing and clinician assessment for diagnosis and possible treatment.

A beta‑2 glycoprotein test detects antibodies to beta‑2 glycoprotein I and is used in the antiphospholipid antibody panel. It helps diagnose antiphospholipid syndrome, which increases risk of blood clots, recurrent miscarriages, and stroke. Persistent positive IgG or IgM results support diagnosis and guide risk assessment and anticoagulation decisions alongside clinical history and other antibody tests.

A positive antiphospholipid IgM means antiphospholipid antibodies are detectable. It can be transient (after infection or immunisation) or persistent. If confirmed on repeat testing after 12 weeks and accompanied by thrombosis or pregnancy morbidity, it supports antiphospholipid syndrome (APS). A lone, single positive IgM has uncertain risk; further evaluation, repeat testing and specialist review guide management and anticoagulation.

A positive IgM indicates recent or active infection; IgM appears early after exposure. A positive IgG reflects prior exposure, longer-term immunity, or response to vaccination. Both positive usually mean a recent infection where the immune response is maturing, but timing, symptoms, and test accuracy matter. Autoantibodies to beta‑2 glycoprotein I (β2GPI) are central to antiphospholipid syndrome (APS), causing recurrent arterial and venous thrombosis and pregnancy loss. They commonly coexist with systemic lupus erythematosus and can trigger catastrophic APS. Associated manifestations include stroke and transient ischemic attacks, thrombocytopenia, livedo reticularis and valvular heart disease. Detection influences anticoagulation and pregnancy management.

A positive IgM result usually indicates a recent or acute immune response—commonly from a new viral, bacterial, or parasitic infection or recent vaccination. IgM appears early after exposure. False positives can occur from cross‑reactive antibodies, autoimmune conditions (e.g., rheumatoid factor), chronic infections, or lab error. In newborns, detectable IgM suggests in‑utero infection since maternal IgM does not cross the placenta.
