Bence Jones Protein (BJ Protein)

A Bence Jones (BJ) protein test detects free immunoglobulin light chains in urine, often via urine protein electrophoresis or immunofixation. It’s ordered when multiple myeloma or other plasma-cell disorders are suspected. Presence of BJ proteins suggests abnormal monoclonal light-chain production and warrants further blood and bone marrow evaluation. A negative result doesn’t completely exclude disease; clinical correlation and follow-up testing are important.

Bence Jones proteins are monoclonal free immunoglobulin light chains (kappa or lambda) excreted in urine, produced by abnormal plasma cells in conditions such as multiple myeloma and related disorders. Their presence suggests monoclonal gammopathy and can cause kidney injury (light‑chain nephropathy). Detection and quantification use urine protein electrophoresis, immunofixation, and serum free light‑chain assays for diagnosis and monitoring.

Detection suggests a plasma cell disorder such as multiple myeloma or related conditions and helps guide diagnosis and monitoring.

Several cancers can cause elevated urine protein. Hematologic malignancies—especially multiple myeloma—produce light chains that cause proteinuria. Kidney cancers (renal cell carcinoma) and metastatic tumors to the kidneys directly damage renal tissue. Solid-organ cancers (lung, colon, stomach, breast) can trigger paraneoplastic glomerulopathies such as membranous nephropathy, and lymphomas or leukemias may also cause proteinuria.

A urine test for leukemia cannot diagnose the disease; diagnosis relies on blood tests and bone marrow examination. Urinalysis can reveal kidney involvement, blood or protein, elevated uric acid or electrolyte changes from tumor lysis, and infections or drug metabolites during treatment. It’s used to monitor complications and therapy effects rather than to confirm leukemia itself; abnormal results prompt further testing.
