A barium swallow test is an X‑ray procedure that evaluates the structure and function of the mouth, throat and esophagus. It identifies causes of swallowing difficulty, pain or choking; detects strictures, hiatal hernia, reflux, ulcers, motility disorders (e.g., achalasia), and tumors; and assesses aspiration risk. It helps guide diagnosis and treatment decisions for dysphagia and other esophageal conditions.

Before a barium swallow: follow fasting instructions—usually no food or drink for about 6 hours; avoid smoking. Bring a list of current medications and allergies; continue essential medicines unless told otherwise. Remove dentures, jewelry and wear loose clothing (or expect a gown). Tell staff if pregnant or breastfeeding. Arrive early and follow the radiographer’s instructions during the procedure.

A barium swallow is usually not painful. Most people feel mild discomfort, throat-clearing or gagging, a chalky taste, and temporary bloating or fullness. Some experience mild throat soreness or constipation afterward. Serious complications are rare. Drinking fluids and using a laxative or stool softener as advised reduces constipation risk. Report severe pain, breathing difficulty, or persistent vomiting to your clinician.

Recovery from a barium swallow is usually immediate—most people can resume normal activities and diet within a few hours. The barium may cause pale, clay‑colored stools or mild constipation for up to 48–72 hours; drinking plenty of fluids (and a mild laxative if advised) helps. Seek medical care for severe abdominal pain, inability to pass stool, fever, vomiting, or breathing difficulties.

Barium swallow is a radiographic test where you drink barium contrast to outline the esophagus and swallowing mechanics on X‑ray; it's non‑invasive, usually needs no sedation, and shows structure and motility but cannot sample tissue. Endoscopy uses a flexible camera passed through the mouth to directly view and treat mucosa, allow biopsies and interventions, usually requires sedation and has small procedural risks.

Barium can be toxic to humans, but risk depends on its form. Soluble barium salts (for example, chloride) are poisonous if ingested or inhaled, causing gastrointestinal symptoms, muscle weakness, low potassium, respiratory paralysis and dangerous heart arrhythmias. Insoluble barium sulfate used in medical imaging is not absorbed and is considered safe. Acute exposure needs prompt medical assessment; workplace controls minimize risk.
