ANCA MPO, in Pimpri Chinchwad

ANCA positive with MPO means the immune system produces anti‑neutrophil cytoplasmic antibodies directed against myeloperoxidase. This finding often indicates a small‑vessel autoimmune vasculitis (for example microscopic polyangiitis) and can cause kidney, lung, skin, or nerve inflammation. It supports diagnosis, helps assess prognosis, and guides immunosuppressive treatment and monitoring for organ involvement and relapse.

A positive MPO means your immune system has antibodies against myeloperoxidase (MPO-ANCA). It suggests an ANCA-associated autoimmune process—commonly microscopic polyangiitis or other small-vessel vasculitis—and can affect kidneys, lungs and nerves. It’s a marker, not a diagnosis: clinicians will correlate symptoms, order blood/urine tests, imaging and often a biopsy, then consider immunosuppressive treatment if confirmed.

No. pANCA describes a perinuclear staining pattern on indirect immunofluorescence of neutrophils; MPO‑ANCA are antibodies against myeloperoxidase. MPO antibodies often produce a pANCA pattern, but pANCA can reflect antibodies to other neutrophil antigens. Labs therefore report both the IF pattern and antigen-specific assays (e.g., MPO immunoassay) because they are related but not identical.

ANCA positivity can be clinically important but its seriousness depends on context. A positive test alone doesn’t confirm disease; correlated symptoms and organ involvement (kidneys, lungs, nerves) may indicate ANCA‑associated vasculitis, which can be severe and life‑threatening without treatment. Further evaluation—repeat testing, antibody subtyping (PR3/MPO), urine, imaging and specialist referral—is required to determine prognosis and need for immunosuppressive therapy.

Treatment for MPO‑ANCA vasculitis begins with rapid induction using high‑dose glucocorticoids combined with either rituximab or cyclophosphamide to control inflammation. Plasma exchange is reserved for severe pulmonary hemorrhage or rapidly progressive renal failure in selected cases. After remission, maintenance therapy (rituximab, azathioprine or methotrexate) is continued for 12–24 months, plus infection prophylaxis and regular renal monitoring.

Myeloperoxidase (MPO) rises when neutrophils and macrophages are activated, reflecting inflammation and oxidative stress. Common causes include acute infections, acute coronary syndromes and atherosclerosis, chronic inflammatory diseases (eg, rheumatoid arthritis), diabetes, metabolic syndrome, chronic kidney disease, smoking and obesity. Elevated MPO is linked to endothelial dysfunction and higher cardiovascular risk, and can increase during acute injury or systemic inflammatory flares.
