Amylase - Serum

High amylase means elevated levels of an enzyme made mainly by the pancreas and salivary glands. It commonly indicates acute pancreatitis or pancreatic duct obstruction (e.g., gallstones), but can also arise from salivary gland inflammation, gastrointestinal perforation, kidney impairment (reduced clearance), or macroamylasemia. Further evaluation with lipase, clinical assessment, and imaging is usually needed—discuss results with your healthcare provider.

A serum amylase test measures the amylase enzyme in blood to help diagnose and monitor pancreatitis and other pancreatic injuries. It is ordered for acute abdominal pain, persistent nausea or vomiting, or after abdominal trauma to assess pancreatic inflammation. Elevated levels can also reflect salivary gland disorders, intestinal obstruction, or a perforated ulcer. Results guide further imaging and treatment decisions.

Treatment for high amylase targets the underlying cause. For acute pancreatitis: hospital admission, aggressive IV fluids, pain control, bowel rest, antiemetics, close monitoring and management of complications; ERCP or cholecystectomy if gallstones or obstruction. For salivary causes: antibiotics, hydration and warm compresses. Stop offending drugs and arrange follow-up with imaging and repeat labs under physician guidance.

Normal adult serum amylase is roughly 30–110 U/L, though many laboratories use reference intervals such as 23–85 U/L or 25–125 U/L. Ranges vary with the assay, patient age, and lab method, so always compare to the reporting lab’s reference interval. Urine amylase has different units and ranges and should be interpreted separately; discuss abnormal results with your clinician.

Certain cancers can raise serum amylase, most commonly pancreatic malignancies (especially acinar cell carcinoma and pancreatic ductal adenocarcinoma). Ectopic amylase production is reported with some lung cancers (notably small‑cell), ovarian tumors (mucinous types), salivary‑gland neoplasms and occasional hematologic malignancies. Renal impairment also reduces amylase clearance. Elevated amylase is nonspecific and needs imaging and tumor markers to determine cause.

Stress can raise amylase in some situations. Physical stressors (severe illness, trauma, surgery, heavy alcohol use) and salivary-gland stimulation increase amylase. Psychological stress commonly boosts salivary (alpha-)amylase via sympathetic activation but may not markedly change serum amylase. Persistently elevated serum amylase should be evaluated to exclude pancreatitis, salivary disease, renal impairment, medications, or macroamylasemia.
